Summary
PT-141 (bremelanotide) is a synthetic cyclic heptapeptide melanocortin receptor agonist derived from the metabolite of Melanotan 2. Unlike phosphodiesterase inhibitors, PT-141 acts centrally through the melanocortin-4 receptor (MC4R) in the hypothalamus to modulate sexual arousal and desire pathways. It is the only melanocortin-based compound to have received FDA approval (as Vyleesi®) for hypoactive sexual desire disorder, and it continues to be studied for its broader neuroendocrine and motivational effects. In pivotal phase 3 clinical trials (RECONNECT), bremelanotide demonstrated statistically significant improvements in sexual desire and reductions in distress associated with low desire in premenopausal women compared to placebo, with effects observed as early as the first dose. Mechanistic studies confirm central nervous system activation distinct from peripheral vascular pathways, with functional MRI data showing increased activation in brain regions associated with sexual arousal. Animal models show dose-dependent pro-erectile and pro-motivational effects mediated through MC4R signaling. Common side effects include transient nausea and facial flushing, generally mild and diminishing with repeated use.
